Mobile Integrated Health
Talbot County’s Mobile Integrated Health (MIH) program is a collaborative initiative between the Department of Emergency Services (DES) and the Talbot County Health Department designed to bring healthcare services directly to residents in need. The program focuses on preventative care, chronic disease management, and reducing unnecessary emergency room visits and hospital readmissions by providing personalized, in-home healthcare solutions.
How It Works
The MIH team, which includes paramedics, nurses, and community health professionals, visits individuals in their homes to assess their health needs and provide essential medical support. These services include:
